Workplace Changes That Trigger a Risk Assessment Review 

A risk assessment should never be treated as a one-time exercise; workplace changes can create new risks that need to be identified and controlled. Here are 8 workplace changes that should trigger a HIRA review.

1. New Equipment or Machinery Is Introduced 

New machine. New risk?

Not always, but definitely a reason to reassess. New machinery can introduce different:

  • Energy sources

  • Moving parts and pinch points

  • Electrical hazards

  • Noise and vibration

  • Automation-related risks

  • Maintenance requirements

  • Access and isolation requirements

For example, replacing a manually operated machine with an automated system may reduce manual handling risks but introduce unexpected movement, guarding or isolation hazards.

Ask:Does the existing HIRA describe the new equipment and its actual hazards?

If not, update it before the equipment becomes part of routine operations.

2. The Work Process Has Changed 

Sometimes the equipment stays the same, but the job changes. Production may increase. A task may be performed differently. The sequence of activities may change. A manual operation may become partially automated.

These changes can affect worker exposure, frequency of hazards and the effectiveness of existing controls. A HIRA should reflect how work is actually performed, not simply what an old procedure says.

Try this:

Ask a worker:

“Show me how you actually do this job.”

Then compare what you see with the HIRA. If there is a gap between the documented process and the real process, the risk assessment deserves another look.

3. A New Chemical or Material Is Introduced 

“It’s only a replacement chemical.”

That doesn’t necessarily mean the risk is unchanged. A new chemical may have different:

  • Toxicity

  • Flammability

  • Exposure routes

  • Storage requirements

  • Compatibility concerns

  • PPE requirements

  • Spill-response needs

The same applies to new raw materials, fuels or other hazardous substances. A change in material should therefore trigger a review of the relevant hazards and controls.

Quick check: If procurement replaces a chemical tomorrow, does your HIRA process automatically trigger a review? If not, there may be a gap in your change-management process.

4. The Workplace Layout Has Changed 

Here’s an easy one to miss.

The machine didn’t change. The process didn’t change. But the layout did.

A relocated machine, changed storage area or new pedestrian route can alter risk significantly.

  • Pedestrians may move closer to vehicles.

  • Emergency access may become restricted.

  • Material movement routes may change.

  • Chemical storage may move closer to ignition sources.

  • Access to equipment may become more difficult.

The HIRA should reflect the current physical workplace, not an old layout.

Try a HIRA walk-through

Take the existing HIRA and physically walk through the area. Don’t just read the HIRA. Compare the HIRA with what you can actually see.

5. A Contractor or Non-Routine Activity Is Introduced 

Routine operations may be well controlled. Then a shutdown, maintenance activity or installation project begins. Suddenly, you have:

New people + new equipment + new tasks + new interfaces.

Activities such as hot work, lifting, confined space entry, excavation and temporary electrical work can introduce hazards that aren’t covered by routine operational assessments. This is where HIRA should connect with JSA and Permit-to-Work systems.

Ask:What is different about this activity compared with our normal operation?

The answer may reveal hazards that require additional assessment and controls.

6. An Incident or Near Miss Reveals Something New 

A near miss may have caused no injury, but it can reveal a weakness in your HIRA. A worker nearly slips. A load almost falls. A vehicle nearly hits a pedestrian. A machine almost starts unexpectedly.

These events are opportunities to ask:

  • Was the hazard identified?

  • Was the risk rating realistic?

  • Were controls actually implemented?

  • Did the actual job differ from the assessed task?

  • Could the same hazard exist elsewhere?

If the answer exposes a weakness, the HIRA should be reviewed. NIST Global’s HIRA guidance recommends conducting HIRA after incidents or near misses as well as before new activities and during significant changes.

7. Existing Controls Are No Longer Effective 

A HIRA may say:

“Engineering control in place.”

But is it actually working?

The machine guard may be damaged. Ventilation may have deteriorated. A barrier may have been moved. An interlock may be bypassed. A procedure may no longer reflect actual work. A control written on paper is not necessarily an effective control.

The better question is:

“Does this control still reduce the risk to an acceptable level?”

When controls fail or become ineffective, reassess the risk and consider whether stronger controls are required. NIST Global’s HIRA framework uses the hierarchy of controls, from elimination and substitution through engineering and administrative controls to PPE.

8. The Organisation or Operation Has Changed 

Risk can change even when machinery and processes remain the same.

Consider:

  • Increased production

  • Extended working hours

  • A new shift

  • Changes in staffing

  • New job roles

  • Changes in supervision

  • New operational areas

  • Outsourced activities

For example, moving from one shift to three can introduce different fatigue, supervision, communication and emergency-response considerations.

Same equipment does not always mean same risk. Significant organisational or operational changes should therefore prompt a review.

When Should You Review the HIRA? 

HIRA review shouldn’t depend only on a fixed calendar date. A better approach is to think in terms of Before. During. After.

Before: Assess What Is About to Change 

Review the HIRA before introducing:

  • New equipment or machinery

  • New chemicals or materials

  • New processes or work methods

  • Contractors or non-routine activities

  • New projects, installations or production systems

The goal is to identify potential hazards before the change becomes part of routine work.

During: Review When Conditions Change 

Risk can change even after an activity has started. Reassess when there are significant changes in:

  • Workplace conditions or layout

  • Workforce or shift patterns

  • Production levels

  • Work methods

  • Supervision or responsibilities

  • Existing risk controls

The key question is: “Does the current HIRA still reflect how the job is actually being performed?”

After: Learn From What Happened 

A HIRA review may also be necessary after:

  • Incidents

  • Near misses

  • Dangerous occurrences

  • Control failures

  • Significant safety observations

  • Evidence that existing controls are no longer effective

These events can reveal hazards or weaknesses that may not have been identified during the original assessment.

Don’t wait for an incident, or the next scheduled review, to discover that your HIRA is outdated. The best HIRA is one that changes when the workplace changes and improves when new risks are identified.

One of the biggest mistakes is treating HIRA review as purely a calendar activity. Periodic review is important, but significant changes should not have to wait for the next annual assessment.

Think of HIRA review in three ways:

Review when equipment, processes, materials, layout, people or work methods change.
